Description
Do NOT just complete this request. I have reported since before 311 system. The sidewalk in front of 3322 Monument Ave is hazardous and the worst i have seen in the city. Repairs were done in front of 3300-3306 just this summer and there was not a request. I fell again carrying groceries into my house because of the sidewalk being side upended. This is my final request before getting my lawyer involved and 12 on your side. Pictures are attached. I want soneone to call me 370-7498.
