    5. This location has been inspected on __10/25/22________(Enter Date). 4_ X_20___ feet of damaged concrete sidewalk due to tree roots. The repair will be conducted according to the rating system and safetyhazard concerns. 4x30 at tree trip hazard at both ends caused by tree roots.
