    D. Rivers assess this service request & spoke with K. Beck onsite. Found: Some areas of asphalt ditch eroded near inlet of the driveway culvert & some spot nearby. Driveway culvert filled with sediment. Solution: Excavate & prep eroded asphalt ditch for restoration as needed. Clean both driveway culverts at 35 Old Mill Rd & 36 Old Mill Rd. There is access point (grate) to clean the culverts. Recommend closing the roadway for safety. Supervisor will make the final determine on best manage practices for cleaning. Lockett to assess for cleaning & asphalt preparation.
