    L.Lockett , This location had 2 of 2 that were inspected for clean and service, 2 of 2 needs service and is marked. Sediment and roots, NW Basins has major root issues, Basin's case has shifted due to the root issues at this location also it is need of repair, Basin Repair has been notify about the issue at this location. Recommend for removed by Urban Forestry as the tree is engulfed by vine and seem to be dying.

    J.Richardson and and S.Crews stated that service request was completed, crew clean location with high pressuse water and vacuum truck. Crew clean sub and basin with high pressure hose and high pressure vacuum.
