    M assessed 5/18/2021 per Lockett...DPU Stormwater contractors PipeVision were assigned to this location, crew cleaned location with high water and vacuum truck. Crew cleaned sub and basin with high pressure hose and high pressure vacuum.

    *previous note error* Per Lockett Location has 2 of 2 basin that need service location has leaves and debris. All basin were inspected at this location. This Location also has Basin grate that needto be clean with high pressuse water jet. Basin at South West corner has broken basin top.
