    AN ASSESSMENT WAS MADE AT THE REQUESTED LOCATION ON THE DATE OF 12/27/2018. THERE WERE NO STORM INLETS AT THE REQUESTED LOCATION. THERE WERE OFFSET CURBS AND VALLEY GUTTERS AND A LOT OF LEAVES THROUGHOUT THEBLOCK AT THE LOCATION. THE CUSTOMER STATED THAT SHE HAD A REQUEST IN FOR ROADWAY MAINTENANCE OF TYHE DEPARTMENT OF PUBLIC WORKS.
